Is used to determine the clients presents state
of health and identify any potential risk
factor that might alter or adversely affect the fitness testing
process. Sometimes medical
clearance from a physician or specialist is required in order
to gather additional information
and/or any recommendations for the purpose of creating a safer
and more effective fitness
program. Possible risk factors include the following:
- Prescription drugs being taken
- Heart problems, chest pains, or stroke
- Increase blood pressure
- Any chronic illness or condition
- Difficult with physical exercise
- Advise not to exercise from a physician
- Recent surgeries or (within the past 12
months)
- Pregnancy (presently or within the past
12 months)
- History of respiratory illness
- Musculoskeletal Injuries presently effecting
you
- Diabetes or thyroid condition
- Cigarette smoking
- Obesity (more than 20% over ideal body weight)
- Increase blood cholesterol
- History or cardiovascular problems in immediate
family
- Hernia or any condition that may be aggravated
by lifting weights
After all permanent information is gathered
evaluated and given clearance to proceed,
informed consent for an exercise test form is given to the
client to read over and sign. The
client is then provided with the following information:
- Purpose and explanation/results of the test
- Attended risk and discomforts
- Responsibilities of the participant
- Benefits to be expected
- Inquiries
- Use of medical records
- Freedom of consent
